[ ] Correct wording when requesting vaccine records from Pediatrician I called my child's pediatrician's office today and told the receptionist I wanted to get a copy of my child's records (specifically how many ear infections he has had and the antibiotics prescribed and all of his shots, the manufacturer and the lot#s). She said I had to come in a complete a form and then it would be up to the doctor what records would be given to me????????? Can someone tell me everything I'd need to know about each vaccine to be able to look them up to see whether or not they contained thimerosal? I want to word everything clearly on the form so that when I get the records they contain everything I need to answer these questions that have been eating away at me for the past year. I believe it varies from State to State - In Washington, we can request the complete file at any time and can receive lab results even prior to our child's pediatrician receiving them. Just go in, fill it out and when they say " what records " write in that you want the papers that list the type of vaccine, date administered, name of manufacturer, and lot number for each vaccine that [my] child has received to date. Then it says " where to " -- list your name and your mailing address. They will send the file to a records handling company if they don't do it right there themselves, and the records company will mail you the documents. SHouldn't be any charge for this at all. I simply called the office and told the nurse that my son's school needed an updated record to complete his immunization form. The nurse said sure and mailed it to me that day! I think they are very used to parents asking for vaccine records for school. Now that summer is upon us, you could use camp as an excuse for requesting the records. Anyway, when I got the form, it was complete with the lot numbers and dates that they were given. It was really very simple..... They said that the file could not leave the office and they would not copy it for me. They also said that they could only give the information to other medical professionals. What's the point of me having to sign a concent form for them to release it to other professionals if I can't even have the file. I think they were messing with me. Guest guest Posted May 6, 2004 Report Share Posted May 6, 2004 My two cents on this: I've gone through this recently and just had to sign a release form and they xeroxed stuff and I picked it up -- BUT looking at it, some info. was left out like phone calls and complete sick visit notes (they have a file system that has very small index card depth pieces of paper that summarize at the bottom the dx or reason for visit and so when they copy it, it copies the summary for like 8 visits on one sheet, but you don't get what the parents say was wrong etc.). The best way to get complete information is through an attorney. We finally bit the bullet and decided to have Jack's med. records and case reviewed and it's business when they subpoena records. I didn't want Jack's peds to know though, so I handled getting them best I could (I'm working the medical exemption for his 5 year shots -- NC state law requires a NC licensed physician to provide the medical exemption, so I've been lobbying our pediatrician who is relatively new in the practice and coincidentally a high school classmate). I know, as I just went through a similar experience when I got my son evaluated for ASD's. My primary pdoc and his partner split. When my pdoc left the practice, the only records he could take with him where the ones he generated (wrote/dictated literally). I went with this doc, as the other doc was the one who gave Liam his Kindi multi vaccine even though he was just starting antibiotics and had pneumonia. So basically most of Liam's " generated " records came from the other doctor (who is a friend of my Dad's, so he was " our " doctor). So I had to sign and get a release from him, then, any other records I wanted, like X-Rays, neurologists, psychologists, etc, I had to go to each separate doctor/provider and get a release signed and a copy of records from them, not my original doctor. His record is still not complete. I have none of his lab work, among other things. None of his vaccine 411, other then he got them, and what they are called. Just what is written down in the notes by each doctor. And, some doctors CHARGE you for this information. I paid up to $75 for one doctor's notes. He said he would send them to another doctor for free, with my signed release. But then MY doctor would legally be unable to give me a copy. Now, when my kiddo's see a specialist, I ask for a copy of all generated reports on the spot. MAJOR pain in the A$$. It is one of the reasons I personally will not seek to find out his exposure levels from past vaccines. I'm just moving forward, though I'd love to know! After receiving the medical records I could and my ped's could see a clear pattern of diarhhea after a certain vaccination. I had brought my son in after this particular vaccination and complained of a possible reaction. He even documented my concern it was vaccine related. I had completely forgot about that visit. After that visit we never had a well child check because we were back every two months with diarrhea chronically and about 8 ear infections in 1 1/2 years. It helped me see a pattern I would not have seen if I did not request them all. It also helped convince my ped's that although he still believes in vaccinations, that my son had " possible " problems with vaccinations. So sometimes a complete picture can help you understand more. Just my two cents worth. I also just told them my son was diagnosed with Autism and that many different doctor's were requesting copies of medical records and asking questions like how many ear infections did he have, and I needed a way to respond to those requests without bugging their office every other day. I had the records by the end of the day. I have never had a problem obtaining a copy of medical records in 12 years of practice. All 50 states will allow a parent or legal guardian to obtain a complete copy of their child's medical file. Of course the physician's office can charge a fee for the copies and require the parent to sign a release, but that is no big deal. If the doctor or some uneducated staffer really does not want to produce the records, we just subpoena them and, we usually ask that the custidian of records appear in person before a court reporter for a deposition on written questions on a Sunday afternoon or Saturday night. The records are miraculously produced after the subpoena and Notice of Deposition go out--go figure. Hope this helps. My best. Last month DH and Liam went to a cub scout camp. Upon arriving at the camp, you had to produce vaccination records. Of course DH didn't tell me this until the day they were supposed to leave! So I called the pdocs office, explained my predicament, and begged them to fax the stuff to DH. DH called me and said " This is very technical stuff, I hope they take it at the camp!!!! DH is going to call me when he gets to work (where the fax is). Crossing my fingers and toes that it is more then just the names and dates!!! The doctor withheld the lot numbers to cover up the fact he lied about thimerosal content... He told their parents that the vax were mercury free, but the truth is, he never checked... To confound the issue, Aventis Pasteur and one other (can't remember which) would not release the thimerosal content when given the lot number... Evidently, from a eugenics standpoint, we're not good or smart enough to be trusted with such information. Us poor dumb parents... Perhaps these smarter folks should start wearing swastica armbands so they are more easily identifiable... This is accurate for certain types of information, primarily information related to mental health issues. That type of information can be restricted and only released to another qualified medical professional. Generally, requesting your complete file to forward to a new doctor, or requesting vaccination records for school, is a good way to do this. Reports of the matters referred to in subparagraphs (A) and ( shall be made beginning 90 days after December 22, 1987. The Secretary shall publish in the Federal Register as soon as practicable after such date a notice of the reporting requirement. (2) A report under paragraph (1) respecting a vaccine shall include the time periods after the administration of such vaccine within which vaccine-related illnesses, disabilities, injuries, or conditions, the symptoms and manifestations of such illnesses, disabilities, injuries, or conditions, or deaths occur, and the manufacturer and lot number of the vaccine. (3) The Secretary shall issue the regulations referred to in paragraph (1)© within 180 days of December 22, 1987. © Release of information (1) Information which is in the possession of the Federal Government and State and local governments under this section and which may identify an individual shall not be made available under section 552 of title 5, or otherwise, to any person except - (A) the person who received the vaccine, or ( the legal representative of such person. (2) For purposes of paragraph (1), the term ''information which may identify an individual'' shall be limited to the name, street address, and telephone number of the person who received the vaccine and of that person's legal representative and the medical records of such person relating to the administration of the vaccine, and shall not include the locality and State of vaccine administration, the name of the health care provider who administered the vaccine, the date of the vaccination, or information concerning any reported illness, disability, injury, or condition resulting from the administration of the vaccine, any symptom or manifestation of such illness, disability, injury, or condition, or death resulting from the administration of the vaccine. The doctor withheld the lot numbers to cover up the fact he lied about thimerosal content... He told their parents that the vax were mercury free, but the truth is, he never checked... To confound the issue, Aventis Pasteur and one other (can't remember which) would not release the thimerosal content when given the lot number... Evidently, from a eugenics standpoint, we're not good or smart enough to be trusted with such information. Us poor dumb parents... Perhaps these smarter folks should start wearing swastica armbands so they are more easily identifiable... If you call the cops and the doc's office calls the cops, guess who gets removed from the property and possibly arrested. And if you persist, social services may join the case against you. I hope there are legal remedies that aren't too expensive. One possibility, a small damage suit against physicians who lose an autistic child's records.
