Guest guest
Hummm Cy, That is perplexing to me that your first exposure to the thought that he might have MPGN 3 would be in this dictation. I would definitely be interested in seeing what the Neph has to say about this and what has led to this conclusion. Please do keep us posted when you get a reply from 's Neph on this. . Everything was expressed in the same glowing terms that I mentioned earlier. One thing was very peculiar however: the dictation states that has MPGN 3. I'm somewhat perplexed because his biopsy didn't indicate which type of MPGN. Previous conversations with the nephrology fellow and the attending indicate that no one wanted to call it, but it fell somewhere between MPGN 1 and MPGN 2 - and they were leaning towards MPGN 1. This reference to MPGN 3 is baffling (not the least because once we listened to the attending discourse as to why he thought MPGN 3 didn't really exist). Hmmmm.... This will be an email later to the neph... Cy

Guest guest
Hi Cathy, MPGN ( membranoproliferative glomerular nephritis) is a very rare disease that is similar to IgAN in many ways - only more extreme. MPGN patients tend to have worse proteinuria, progress faster, have low C3 and C4 and there is a small handful of other symptoms (i.e anemia can arise independantly of the usual renal means, immune deposits can also be found on the retina). MPGN 1, 2 and 3 are different varients that are distinguishable upon biopsy. Knowing which can give you a heads up as to how fast it progresses and what other symptoms arise. MPGN 2 is the most aggressive varient - the rate of graft loss due to recurrence is 95%. It is also occationally associated with blindness due to retinal deposits. MPGN 3 is the rarest - and very little is known about it.
