Histologic grade refers to how much the tumnor cells resemble normal cell. The lower the grade, the more the tumor cells resemble normal cells. Nuclear grade refers to the rate at which the cancer cells in the tumor are dividing to form more cells. Cancer cells that divide more often are faster growing and more aggressive than those that divide less often. The nuclear grade is determined by the percentage of cells that are diving. The lower the grade, the more normal. I would definitely NOT to do without the chemo or radiation UNLESS your onc suggests leaving the radiation out. I had friends that opted to switch from chemo to an alternative, they each had better odds than I did, and they all died within a short time of each other. But its a decision only you can make. I have just asked for and recieved a copy of my pathology report and one part of it says " High Nuclear " . Can anyone tell me what this means?. I am told I have a " small Cancer " but yet this High Nuclear does not sit well with me. I was also told that I would have to have radiation AND Chemo. How do they know this already? Is it because I am estrogen,progestrone, and HR2 negative? Is there any other option besides Chemo? Guest guest Posted November 18, 2006 Report Share Posted November 18, 2006 Hi Dee, I have just about the same diagnosis as you! Small cancer, Stage 1, ER pos, Her2nu neg, Grade 3 (highest) invasive. I had a lumpectomy, chemo and radiation. Although the staging is important, the grading is equally important in making decisions. Like nne said, the grading tells you how agressive the cells are that were invasive. My decision to do chemo and radiation was based on that finding. My opinion was that I needed to do all that I could to prevent a recurrance. There are no guarantees of that, regardless of what I do, but for my family and my own piece of mind, I needed to do whatever was provided for treatment. I was a stage 1 idc 1cm(small)but a grade 3(very aggressive) HR2neg(double whammy)This why we have all this extra stuff 8 months chemo ac(4months) and taxol(4months)radiation 5days for 6weeks. I am going stay " vitamined up " . There are many reports regarding, " antioxidants interfering with chemo...sonmewhat true. Chemo is more effective for estrogen negative tumors. I had the lumpectomy May 1st. Then wawas told the results the 8th. THe 11 went back in and had surronding tissue removed and nodes (sentianl Ones) removed. The 17th found out 1 node was positive. That is why I had to do Chemo and radiation. If your nodes are clear I don't know why you need Chemo. But if you do hang tough it is a hard road! I am done with Chemo and am now doing my rads! The radiation is a breeze compared to the chemo!! Good luck and keep us posted and we can let you know what you might experience along the way!!!!
