As far as tips go, definitely send certified mail/return receipt

requested incase you have any problems with them saying they didn't

get it; get your ped. to write a letter of medical necessity; the

appeals packet from CT was very helpful. I can't remember which

letters I used; however, I tried to find ones that in the subject said

UHC appeal successful or approved. Good luck in your appeal. Oh one

more tip, look at your UHC plan and use anything you find in there to

help win your appeal because they can't deny you of something that

your plan says is included. I used a lot of the durable medical info

from our plan.
