I am not his doctor of course, but we were told to leave on the helmet with low grade fevers. My daughter had two fevers since she had the helmet, one around 101 and we left on the helmet and then one between 103 and 104 and we took the helmet off with it. But not even at all times, only when she got really hot and we needed her to cool off a little. We put it back on every time after 3 or 4 hours.
