However, I will tell you for this first appeal they are saying its not covered or is experimental neither of which are true. And they actually told the doctors office it was covered in advance. I wasn't successful contacting people on the insurance page, so I am starting a new post on the topic. My daughter (7 months) is in her second week of her DOC Band. It is important to make the script notes that your daughter has been diagnosed 1st with congenital torticollis and 2nd brach or Plagio. It also HAS to say that you went through PT and repositioning therapy for at least 8 weeks, unsuccessfully, my doc had no problem writing this, however my son did respond. Anyhow, I know this is probably not new info, but it worked when they rebuttled the pre-authorization for my son. Good Luck! And we too were told by our doctor that he would be surprised if it weren't covered. Well, it's not. When we called headquarters of the place we went through (American Prosthetics and Orthodics) they told us more and more are not covered.
