tenotomy surgery advice PLEASE!

Hi,

's tenotomy (her second) is scheduled for August 20th. They

will be doing it in the OR at Sinai Hospital with light sedation

(whatever that means???). I got her pre-op papers in the mail today

and literally started crying and felt sick when I read the " no

eating " rules!!!! Here is what it says:

**Children under the age of 6 may have clear liquids

(water,Pedialyte,diluted apple juice,ginger ale) up to 2 hours prior

to surgery. No breastmilk up to 4 hours prior. No milk or solid

food up to 8 hours prior.**

Her surgery is scheduled for 1:00pm. This means she cannot have any

solids after 7 am (sometimes she's not even up yet then!) and I

cannot nurse her after 9 am!!! How in the ____ am I supposed to do

that to her??? She always nurses first thing in the morning, then

has infant cereal around 8/8:30, lunch at 11:30 and nurses for nap

at 1:00...she also nurses if she wants anytime during the day. She

is NOT going to understand why she can't eat and especially nurse.

I know some of you have gone through the tenotomy with older

babies. PLEASE give me advice! I am really feeling sick about this.

uhhh... do the tenotomy in the morning? Why on earth would they schedule a

child that late?

Check out the LLL website - there's some white papers and articles or study

information about bm and surgery. I found them once for someone else. I

was always under the impression that bm was actually considered a clear

fluid anyway. We all know that 15-45 min after an infant has nursed it's

all gone from their tummy because that's how fast it's digested and they're

hungry again lol. The 4 hour rule actually is pretty lenient as compared

to what some parents are being told. I've heard 8 hours for all babies

before, FF or BF. I know there's some info on the LLL site about not only

what reality is with anesthesia and BM, I'm sure I saw some articles with

tips or suggestions for getting through surgery with a nursling.

I hope it goes easier than you expect - wouldn't that be nice? Why don't

you just demand that they do it first thing in the am. Or about 4 hours

after her morning nursing?

Hi Kori,

You and my DH had the same reaction...ask for a different time.

Evidently that's not an option. Dr Herzenberg's secretary even said

she always does the little ones in the morning but that is the only

time she can get the OR that day and she wants to have the

BEST pediatric anesthesiologist because of her heart defects.

I'm definitely checking LLL, didn't even think about it and I'm

friends with a LLL leader!

You feel sick, wait to you hear my story........my 1st son's tenotomy was done

under general when he was 3 mo. old. They told me no solids. Just, " No

solids. " Well, the kid was 3 months old, he wasn't ON solids, kwim? So I

breast fed him like normal up to the time they said " no food after....X o'clock "

Of course there were hours there of him starving, then get to surgery and the

gas-man says, " When did he eat last? " I tell him and he freaks out that I

breast fed the child saying milk is a solid and he refused to gas my boy until

he went another several hours with no food. My baby in the hospital screaming

bloody murder starving to death adn people looking at me walking him in the

halls like I was a Demon Mother putting curses on him. It was terrible.

There's no easy way to starve your baby, plain and simple but at least you won't

screw up like I did by mis-understanding their directions and make it worse.

was 7 months when he had is second tenotomies under light sedation (at

Sinai also). We basically had to change his schedule up for the day. It's

workable, trust me.. My baby loves to eat so I worried about this as well.. I

made sure he got whatever he could have just before that time allowance was up.

Oh my! What a horrible experience.

Yes, I certainly hope it will go better than that. Why did they do

it under general? s' first was done right in the office with

local novacaine.

Yes. They siad they don't like the risk of movement if they only

restrain. As much as I don't want her " drugged " , I don't want her

to go through the trauma of being held down.

I can't comment on the tenotomy but I can tell you that my son had

to have an MRI w/ contrast when he was 3 years old. This required him to be

sedated and I was given those exact instructions except his appointment was

scheduled early in the morning. It wasn't bad at all but 1:00 p.m. is late!

Can't it be scheduled for another day early in the morning?

Mom to 12/17/98 and

Christian 1/30/04 (unilateral clubfoot) .

Ya that's a tough call - but you want to make sure it's done right the first

time.

I know this wasn't directed at me but I'll chime in that at 3 months old,

had a hard time " coming to " , he was very hysterical for a long time and in a lot

of pain, but please keep in mind this was not a Ponseti doctor and was the dr.

that messed 's feet up to begin with. One side note however, had a

lot of nightmares for about three weeks following the operation.

He was very very cranky when he first woke up... It took about 30 to 45 minutes

to get him somewhat settled. The rest of the day he was still fussy but after

that he was fine...

I work on a Same Day Surgery unit. We have many pediatric patients

that are having dental, throat, ear surgeries. Very often they wake

up not " about themselves " . I don't know how to explain it. They cry

and it's often not do to pain. They usually go back to sleep for a

little while (30 minutes to few hours) and then wake up as different

little humanbeings! It just seems to be how the anesthesia affects

them. I've even had a few teenagers who wake up and cry and they

don't know why and can't stop for a little while. Not trying to

scare anyone, just know that if your child wakes up like this, in my

experience, this seems like a common reaction.

Devlin was only 7 weeks old when they put her under general

anesthesia for her tenotomy. We had the same instructions with the

feeding: no formula after (I think) 6 hours, no breast milk after 4

hours and then no liquids (they had supplied me with some glucose

water) after 2 hours. I stuck to that schedule, waking her up just

to make sure she got whatever she was allowed at that point. She

actually never complained about the hunger which AMAZED me because

she had always been such a hungry baby (this was one of the first

times she ever had formula though, so maybe it " stuck with her " long

enough to make her happy).

Now, we had the fortunate situation of being one of the first

surgeries of the day so she was only " starved " for a few waking

hours. It's a shame you can't be scheduled for the first of the

day...that really helped.

Devlin was fine when she woke up, just a little groggy and hungry.

After she had glucose water and kept it down they let me nurse her

which put her right back to sleep. But, maybe because she was so

young we wouldn't see any type of reaction. I, on the other hand,

wake up in a terror from general anesthesia and I cry uncontrollably

for about an hour---it's silly because I can laugh about it later but

at the time it seems like everything is just so terrible all I can do

about it is cry...different people, different reactions.

Marilouise,

Zoe had her tenotomy at 2.5 months. Hers was scheduled for 8am, so I

got up and nursed her 3:30-4am and then kept her asleep until they

took her from me for the procedure. It is still 3 weeks away. Is

there any way to reschedule for an earlier time?

Thanks Pam! I am making myself sick about this already :( I know I

need to get my emotions in check because she will know if I am upset

and that will not help things.

Thanks Pam! I am making myself sick about this already :( I know I

Hi,

The time thing is complicated. Dr Herzenberg is away until August

18th. The 20th was already a busy OR day and he does all his

surgeries after his CF clinic. She is his first surgery at least!

Because she has four congenital heart defects they want her to have

the *BEST* anesthesiologist so they are working with his schedule

too.

My older kids start Kindergarten on the 24th and my DH is a school

teacher so he can't take off easily -- especially in the first week

of school! My family and my DH's family can't help out watching the

older two (driving to our house so they don't miss school, etc). I

don't want in casts for weeks longer than necessary. The

driving back and forth to Sinai is tough. I know lots of parents

have done the driving or flying -- a LOT of them farther than us --

but I am trying to keep my older two in as normal a life as

possible. All of us have kinda been controlled by 's medical

appts since birth.

Anyway, I am not trying to make it seem like our life is horrible

becasue I know that things could be MUCH worse but that's why I am

not pushing for a different surgery time and just trying to figure

out how to deal with what we have.
