There were a bunch of questions asked and I took all photos I had from her birth until now to show him. Some interesting things came of our discussion: 1. With 's first casting series I was told to soak the cast off at home the evening before her next casting. Dr. H said that should not be done since there can be a lot of progress lost in that time frame. 2. After 's tenotomy, she was only casted for 2 weeks...seems it should have been 3 weeks. 3. Her DBB angles were set VERY wrong!!! Her " normal " foot has always been at neutral (0*) and her club foot set at 30*. Dr. H said it should have been 40* for normal foot and 70* for club foot. That's a HUGE discrepancy. 4. He asked when I started noticing the relapse. I told him I hadn't noticed any relapse, that what he saw was the best her foot has ever looked. Well, is back in a cast. Dr. H is predicting 3-4 castings and then back to the DBB although hours per day have not been decided. He says he will make that decision based on how her foot reacts to the castings. She has already shown that this cast will not slow her down!! She was off crawling as soon as she touched the floor. She keeps pointing to her cast as if saying " What's this thing? " . So now a question for those of you who have been through the casting either more than once or when your child was mobile...how do you manage bath time? Since 's last cast came off just before she turned 3 months she was not sitting up yet or anything so sponge baths were not a big deal. Now she is sitting, crawling, etc. Any ideas? Thanks! Guest guest Posted July 9, 2004 Report Share Posted July 9, 2004 Huggies wipes (in the refill bad) They are quite wet, and are excellent for " bathing " . the ripples actually make them work better than a washcloth. We actually do this once a day and bathe every 2-3 days. As for the hair ... if you can get her to lay down on the counter and you have a hand sprayer, you can wash it that way (with her head over the sink), otherwise I just used a wet washcloth. Kai did not have much hair any of the times he has been casted (3 time - 4 casts firs time, 2 the second, and 4 the third) so we just wiped his head well with a wet cloth. Sounds like her last doc was a bit of quack ... how can you learn a method and then screw it up that bad?? Guest guest Posted July 9, 2004 Report Share Posted July 9, 2004 What my hubby and I did was he held Tyler's legs and we laid him in a small amount of water in the tub and I quickly washed him. We did this every few days and just wiped him off with a washclothe in between baths. Tyler was crawling alot when he had his last set of casts so this is the only way that we could get him bathed and it worked very well. He didn't like it much but like I said we had to do it quickcly. That is the worst thing about the casts I had a hard time keeping them clean and him. I used to put socks on over his cast when he wasn't wearing his sweat pants. Finally I just let him go they got dirty but it was too much work trying to keep them clean. Another great thing I had my doc do was to put fiberglass over his plaster, this added durability to them. By the time he would get the plaster ones off the knees would be all pushed in. Another thing that I would tell you is to keep an eye on the toe position, my son's casts slipped and I did not know it and he lost all of the correction from those casts. Guest guest Posted July 9, 2004 Report Share Posted July 9, 2004 Here's how I handled it - I had an infant to six months in casts, then the same child age 21 to 29 in casts again. I took a big plastic baby-bath tub and drilled it full of little holes so it became a huge sieve (colander?) like you drain your noodles through. I'd put a bread sack or something over each leg then lay him in that tub where I could pour water over the rest of his body . The holes kept the water from pooling so it didn't accumulate and get his casts wet but I was still able to wash his privates well, etc., and do his hair. For outdoor play I put big tube socks over his casts to keep them clean and to keep dirt from getting in at the thigh or toe areas. She really is a wonderful doctor I just think maybe she's not the best *club foot* doctor. Right now she knows nothing about our visit to Dr. H today. Feel like I snuck behind her back but I DID call and ask her to call me and she didn't SOOO... I'll keep you posted! Still waiting to see if Dr. Ponseti replies to my email. I made sure I gave a really good soaking bath before we went today figuring she was going to end up back in a cast so now I have at leats a day to figure out what I'll do. Dr. H did put a layer of fiberglass over the plaster...a last minute thought on his part. I'm glad he did! has already worn smooth a few parts of her cast as the main floor of our house is mostly ceramic tile so she's been dragging the cast across the tiles. Luckily we are ripping the floor up this fall so i don't care what happens to the tile - LOL! Guest guest Posted July 9, 2004 Report Share Posted July 9, 2004 This is sometimes the case with ortho's. Most were taught that surgery is the only way to correct clubfoot. Some come to the Ponseti method and use it 100% how it was intended. Others come to the Ponseti method in whatever bits they feel like using - they perhaps do not realize the amout of time and research that was put into it - and think that some parts of it wont work or that they can do better. These doctors almost always fail and return to surgery. Some doctors have heard about the Ponseti method and use casting same as they always did and have heard how the DBB helps and use it too thinking that that is the only difference, not realizing that the real difference is in the manipulation. They also almost always fail and end up in surgery. My suggestion would be to let your old ortho see the difference in 's feet after Dr H has corrected them. It may change her mind about using the method properly and getting full training in it. Dont feel bad about 'going behind hr back'. Any good doctor understands the need for a second opinion and will not resent it. You as a parent have to do what is best for your daughter and not worry about the feelings of the doctors. that may sound mean, but it really is not. Your job is to parent, their job is to provide medical treatment. If their medical treatment does not work, it is your obligation as a perent to seek out treatment that will.
