Numbers and Insurance Companies

Does anybody know what numbers insurance companies usually look for

when writing an appeal letter?

I would think Cranial Vault. I would provide them all the information

you can. If your Starband provider has a form they filled out then

I'd request a copy and send it. The more you give, the better in my

opinion.
