Pat,

Just e-mail Cath Sherrer at CSherrer@... and include all

of the things that you would have filled in on the order form (name,

address, name of book, copies, etc.). She will e-mail you back if she

needs more information, otherwise I would imagine she will mail a

copy of the book to you with an invoice which you can then pay by

check.
