I have a question... It seems that as I read messages on the board, the moms have data/measurements on their child's plagio. Yesterday my son received his Hanger helmet and the only measurement that we have been given is an index of height to width of his head as it compares to an average babies head. What measurements should we ask for as it relates to brachy? Thanks so much! For more plagio info
Guest guest
I guess I have a question on this as well. The STARband center I use has measurements for the cephalic ratio by both age and sex. What is considered severe for a girl versus a boy is different, and also there is a difference when a baby is younger than 6 months and older than 6 months, and then I think the next age division happens at 12 months. I wish I had a copy of the information to put numbers to what I am saying. It also says that normal is considered to be +/- 2 from the standard deviation. The orthotist told me that 95% of babies fall into that range. Has anyone else seen this? For the graduated brachy babies out there, what was the cephalic index before and after treatment? Thanks! Guest guest
Yes, most babies fall within 2 SD, but honestly I don't know how that maps to numbers. There are some differences based on age and gender, but I think that is pretty minimal. I think the numbers Molly provided are very usefull (unless you only know the SD). The cephalic index is width/length (side to side/front to back). My daughter started at about 98% (not sure how many SD that is) and ended at about 86%. 86 is still higher than average, but her head looks pretty good, and it was a lot of improvement. If someone knows how to map the measurements to the standard deviations I would like to know. Our doctor (cranial facial plastic surgeon) recommends banding at 90% or above. I'm guess that might be about 2 SD, but I really don't know. There was one paper I found online that said 2 SD above normal was 91.4. thanks. -christine > > I guess I have a question on this as well. > > The STARband center I use has measurements for the cephalic ratio by both age and sex. Guest guest
, Thanks for your response. I did some searching and found this on a website for Aetna and their policy for banding. I don't know how accurate it is. Sex Age -2SD -1SD Mean +1SD +2SD Male 16 days to 6 months 63.7 68.7 73.7 78.7 83.7 6 - 12 months 64.8 71.4 78.0 84.6 91.2 Female 16 days to 6 months 63.9 68.6 73.3 78.0 82.7 6-12 months 69.5 74.0 78.5 83.0 87.5 It looks very similar to the list my ortho uses. However, my ortho would like to get to 87% and I am hoping we are there this week at his appointment on Friday. Guest guest
Ok, all of this discussion is really helpful!!! It seems that there is not a standardized way of measuring Brachy? Is that fair? I called my orthotist back and asked for my son's measurement and it is 93 however, he categorized that as moderate. When I asked if there was a standardized rating system to determine slight, moderate and severe his answer was no. Is every company different? If so, any advice on what my goal should be with treatment? My ortho says high 80s. >> I guess I have a question on this as well. > > The STARband center I use has measurements for the cephalic ratio byboth age and sex. Guest guest
Since our doc recommends banding above 90, his goal is to get in the 80s, but the lower the better. My dd was at about 88 and I was ready to quit (after 4 mo). He recommended we continue until she outgrew the band, and I'm glad we did because she got down to 86 a month later. Since with brachy you generally have extra width you won't end up with a " normal " number like 80. -christine sydney 23 mo starband grad >> I guess I have a question on this as well. > > > The STARband center I use has measurements for the cephalic ratio > byboth age and sex. Guest guest
Another question, it also seems like some doctors have a measurement where they recommend banding. Is that true? My experience was totally different... my pediatrician said that my son would grow out of it and I requested to see a surgeon and even the surgeon said let's watch it for a month. I proactively took my son back in for a followup and still did not get a definitive reco to band. He really left it up to us. My concern is this, I just don't want parents who do not know what they do not know to make the wrong choices out of lack of definitive recos from the doctors that they trust. Is there any advice anyone has on getting doctors to be more proactive on this topic and with treatment? >> I guess I have a question on this as well. > > > > The STARband center I use has measurements for the cephalic ratio > > byboth age and sex. Guest guest
At what point doctors recommend to band varies from doctor to doctor. Sometimes it is dictated by insurance requirements. Other doctors look at the whole picture - i.e., the asymmetry of the head might be small but is there is a lot of ear or facial asymmetry they will recommend banding. Unfortunately some doctors are still clueless and will say that it will round out on its own regardless of what the measurements are. Guest guest
Unfortunately a lot of doctors are anti-banding and try and convince every parent the child will outgrow it no matter how severe the headshape is. I really don't know why, since if you spent 5 minutes on the OlderPlag group on you would see they obviously don't always outgrow it. Even here several times a year there are posts from parents of all different age kids wishing they had known to do something before age 2. I think the only options sometimes are to force your doc to write the Rx even if they aren't in agreement, or to switch to a new doc. There are definitely lots of parents who get bad advice from their doctors on this. It is sad that someone should have a deformed head because their doctor did not " believe " in banding. I can tell you my husband's head is still flat and he's 43. It's not bad, but I'm sure it would look nicer if he had a rounder head (he's from India where back sleeping has been the norm for some time). Even my ortho told me his nephew (age 12?) had a flat head and his scuba goggles keep slipping off because he doesn't have the curve to his head that normally holds them on. -christine sydney 23 mo starband grad >> I guess I have a question on this as well. > > > > > The STARband center I use has measurements for the cephalic ratio > > > byboth age and sex.
