Pacificare denied coverage- what's my first step?

Hello! I just received word this morning (from Cranial Tech) that

cranial bands are not covered at all by our insurance. I looked

through the files and found the appeals letters, but they all

reference denial letters...etc. which we do not have since it was a

verbal thing. What should my first step be? Tiernan will not be

banded for another month, but I'd like to take that time to get

insurance in order. We have Pacificare; I'm not sure if it's PPO or

HMO. It's open access if that makes any difference. Either way,

HELP!!
