Ive never heard of taking lantus twice a day. i was always told that lantus was

a long lasting insulin that last 24 hours and should cover you between means and

such. i know that with my daughter depending on when i give her her lantus she

can run low a certain times of the day when it peaks.

It's perfectly in order to take Lantus twice a day usually splitting the

dose between evening and morning. Lantus peaks at around 6 - 8 hours after

being injected and can last 24 - 36 hours. If lows are being experienced in

the afternoon or evening then splitting the dose may solve that problem.

I thought that Lantus stayed at flat rate throughout the day (24 hours)

- does it peak?

Lantus may have a slight peak at 6-10 hours and may drop off before 24 hours

or may last longer than 24 hours.

From the Lantus package insert:

" LANTUS*®* (insulin glargine [rDNA origin] injection) is a sterile solution

of insulin glargine for use as an injection. Insulin glargine is a

recombinant human insulin analog that is a long-acting (up to 24-hour

duration of action), parenteral blood-glucose-lowering agent. "

The key phrase is " up to 24-hour duration of action. "

Also from the package insert:

" The median time between injection and the end of pharmacological effect was

14.5 hours (range: 9.5 to 19.3 hours) for NPH human insulin, and 24 hours

(range: 10.8 to >24.0 hours) (24 hours was the end of the observation

period) for insulin glargine (Lantus). "

Note that the range for insulin glargine (Lantus) is 10.8 to >24.0 hours.

From the package insert regarding dosage and administration:

" LANTUS is a recombinant human insulin analog. Its potency is approximately

the same as human insulin. It exhibits a relatively constant

glucose-lowering profile over 24 hours that permits once-daily dosing. "

Notice that it says 'that permits once-daily dosing,' not that it can *ONLY*

be given once a day.

You should always check with your doctor, and follow your doctors directions

for dose and frequency. If *anyone* is going to second guess what your

doctor says, it should be your pharmacist. Just because faceless strangers

on the internet say they have never heard of it doesn't mean your doctor is

doing something wrong.
