I'm playing with the HOMA tool available from

http://www.dtu.ox.ac.uk/index.html?maindoc=/homa/

It calculates IR and beta cell function from insulin or C-peptide levels. I

tried putting in numbers until I got IR of 1 = insulin sensitivity of 100%,

which would be considered normal. Here are some possibilities:

BG Insulin % beta cells

100 7.5 75.9

80 8 123.8

65 8.5 195.9

Then I tried to get 100% beta cells, where S = insulin sensitivity:

BG Insulin % beta cells S

80 5.8 99.9 136.5

85 7 100.3 111.5

88 7.8 100.6 99.3

So according to this model, with normal beta cells and normal IR, you'd have

a fasting BG of 88 and a fasting insulin level of 7.8 microunits per ml.

I'm not sure what anyone can do with this information, but it allows one to

very roughly estimate IR and % beta cells if you have fasting BG and fasting

insulin or C-peptide levels.

It just occurred to me that this is very close to the 85 mg/dL that

Bernstein says most nondiabetics have at almost all times.

I used the model to show what happens if the pancreas output saturates. With

the parameters I inputted, IR increased more rapidly in a decidedly nonlinear

fashion for BGs above about 220.
