Guest guest
Your hospital can provide you with a copy if you would like to have one.I know when I preregistered for admission I was asked if I had one or wanted one. > Does anyone have a copy of a living will that they could send me. Guest guest
This hospitals always have a living will form. You can get one from them and fill it out and have it witnessed. Another alternative is to buy Quicken Lawyer 2003. I used that to do a financial power or attorney, regular will and living will. The financial power of attorney must be notarized though. Guest guest
The Living Will should be witnessed by 2 separate witnesses and a notary public as well (this is the law in the State of Florida anwyay). It's a good practice to have the document witnessed even if not required in your State. Re: living will This hospitals always have a living will form. Guest guest
Becky...the form I used is called " Five Wishes " and it was given to me by the hospital during my pre-surgery consult. You can see a sample at http://www.agingwithdignity.org/5wishes.pdf but it won't print. This document is widely used and as legal as can be. Hope this helps. > Does anyone have a copy of a living will that they could send me. I > am not scared of this surgery, but would like to have everything in > order before I go. Guest guest
I filled out the Five Wishes form and brought it with me to the hospital. Two nurses in the pre-op unit witnessed it for me, made a copy for the surgeon, and gave the original to my wife. > > Does anyone have a copy of a living will that they could send me. > I > > am not scared of this surgery, but would like to have everything in > > order before I go. Guest guest
Good Catch! I never thought about it being different in other states but logically it would be. In WI it just needs to be witnessed by two. The financial power of attorney does need to be notarized also because money and assets are involved.
