Does anyone have a copy of a living will that they could send me. I

am not scared of this surgery, but would like to have everything in

order before I go. Thanks

Your hospital can provide you with a copy if you would like to have

one.I know when I preregistered for admission I was asked if I had

one or wanted one.

This hospitals always have a living will form. You can get one from

them and fill it out and have it witnessed.

Another alternative is to buy Quicken Lawyer 2003. I used that to do

a financial power or attorney, regular will and living will. The

financial power of attorney must be notarized though.

The Living Will should be witnessed by 2 separate witnesses and a notary

public as well (this is the law in the State of Florida anwyay). It's a

good practice to have the document witnessed even if not required in your

State.
