And with being on medicaid, I'm thinking it is going to be very hard getting approved. It may take me years to get where I need to go. Not a fun thought at all, but in all honestly, thats the way it is. And about my surgeon, I think you may be right saying he really doesn't know much about the surgery, but he has always kept my best interest at heart. He is the only one who has stuck by me these long 12 years since I got sick right after having my twins. I know deep in my heart if he thought it was the right thing to do, then he wouldn't hesitate to help me. He is a great guy. And I don't think he is afraid of losing me to another doctor, if it would help me, he wouldn't care at all. Thanks for the suggestions. They are great ones. It's easy for a dr to dismiss something they probably know nothing about. I would think Dr's don't like losing their patients to other dr's. I know that when I had my surgery she had gotten approval from medicaid in one the the midwest states for a patient. She may have already had a patient from your state and had gotten approval before. don't gibe up brcasue you have Medicaid. In my mind......This is insane! An epidural because of all the narcotics you are taking? I honestly have never heard of that........yikes! I don't mean to scare you or anything, but YIKES! OK, let me try to understand this....if you do the " detox " , then they will be able to use general on you? Or is it even IF you do detox, they still have to do an epidural? Sorry, I'm confused with this, please forgive me. Have you ever had an epidural before? The only time I did was when I had my c-sections.....never again. When I had knee surgery the ortho doc suggested an epidural and I refused. Good Luck, and let me know the reasoning on this. I'm afraid I just don't get it. I'm very concerned about this. I am having surgery in six weeks and have to get myself off of the narcotics I'm taking so the pain medication will work for me. The dr said that with the amount of narcotics I'm taking now regular pain management efforts will not work so I'll need the epidural. Guest guest Posted January 20, 2002 Report Share Posted January 20, 2002 I had a distal resection of the pancreas in 1994 was taking 8 Vicodins a day plus had been in the Er every 7-10 days getting >1000 mg. each pop. They did the epidural and after I woke up I was in excruciating pain-what they're not telling you is that the epidurals for pain management are not successful in everyone. There is no medical reason that I know as a family physician that you need to detox prior to surgery. First of all the withdrawal will be horrible. Second, you probably need the pain meds since I assume you have chronic pancreatitis-you need to live 6 wks in excruciating pain,possibly nausea and vomiting and malnutrition?-not a great way to go under the knife. They need to get a pain management doc to guide them through the pre- and post-op period. If they think this will cure your pancreatitis problem then they can try weaning off the meds after you've recovered from surgery. You need a second opinion-I think your surgeon is uncomfortable with pain management and has preconceived and probably incorrect assumptions about narcotics. Guest guest Posted January 20, 2002 Report Share Posted January 20, 2002 Dr , I do not have CP, but I have chronic pain radiating from my pancreas ever since I had acute pancreatitis in June. The pain persisted, a CT was ordered and a cyst was found. I have been ill ever since. I've had pain ranging from mild to severe, vomiting, over a dozen trips to the ER and five admissions. So that brings me to surgery. The surgeon told me that with the amount of vicodin 10-650's I take that pain management after surgery would be very difficult. He wants me to see a pain management doctor as soon as I can to work out the issues I have. He wants me to get off the vicodin and onto a non-narcotic. Personally, I want to get off the vicodin. My pain has been ranging from mild most of the time to moderate some of the time. I do not need the vicodin for the pain, I need it because I am physically addicted. I know very little about epidurals. I will have to discuss it in depth once I see the pain management doctor. What causes them not to work? What other options are there for pain control?
