Guest guest Posted September 12, 2004 Report Share Posted September 12, 2004 Hi, our 14 years old son (Lennox-Gastaut) is going to start fasting tomorrow and later on the Ketogenic Diet. We have came across a problem: our son's weight is already 53 kg (117 lb) and therefore he needs a lot of protein (48 g = 0,71 x 53kg). This means that we can't build 4:1 diet for him. Here in Finland we have very little experience on the diet -especially with teenagers. According to our doctors and dietician our son is probably the first teenager in Finland whose seizures are tried to control with the diet. Is there any possibility to succeed with the ratio 3:1? The really important thing is to get the calories right. How many calories has your son been started on? The calculation of protein doesn't seem quite right. . . . 0.71 x 53 = 38 not 48g At this level we were able to start with a 4:1 ratio with minimal carbs. If you would like to post all the details such as calories, protein, carbs etc people here may be able to help you. Calories are 1500 kcal/day, 144 g fat/day, 38 g protein/day, carbs 10 g/day, 3 meals /day. The problem is that if we take 4:1 ratio there can not be any carbs at all (with 38 g protein), that's why we are starting with ratio 3:1. (1*4kcal/g*38g+4*9kcal/g*38g=1520 kcal is already over 1500 kcal) so the basic question is the amount of protein. Guest guest Posted September 12, 2004 Report Share Posted September 12, 2004 Hi Jaana At first glance, the calories seem very low. My daughter is 53 Kg also, but she is on 1720 calories. The dietician wanted me to put up Claire's calories, but I wouldn't because she is doing very well at that level. Claire is mobile, but not active. Unless your son is completely immobile, I can't see that 1500 calories would be enough. If you leave the calories as they are, you could reduce to 7g carbs per day. That would increase the ratio to 3.2:1. Claire's dietician said that carbs could be reduced to about 1g per meal if needed. However, if I were you I would get another opinion about the overall calories. I would not advise you to reduce the protein lower than 38g per day. That is the absolute lowest he should have. That is about what Claire started on, but after 3 years on the diet I have been able to reduce her ratio and increase protein. She now gets 46.5g protein and 7.5g carbs per day. I can't find it in the book at the moment but I thought I read somewhere about teenagers sometimes being started on a lower ratio? Anyway there is no reason to think it won't work. You are probably okay with .71. What you haven't said is the total calories per day which is the most important number of all. My daughter was on 880 calories per day and getting 18.4 gm of protein and 3.6 gm of carbohydrate per day at a 4:1 ratio (if I didn't make a mistake). The ratio whether 4:1 or 3:1 is a simplification anyway to permit calculations. What you have to look at is weight gain or loss and ketones in the urine, also signs of too high ketones. The most important things are to read Dr. Freeman's book at least 5 times cover to cover and keep it in you bag at all times; also you will need a very good vitamin and mineral supplement. We use Wholelife in California and they ship to Canada. You may be able to get them to ship to you in Finland if you can't find anything suitable there. It should be a good game on Tuesday!
