I doubt an out-of-hospital myocardial infarction.

Probably as simple as not in hospital MI in other words a heart attack that

the patient did realize he had at home versus going to the hospital.

The patient is in atrial fibrillation.

The doctor says:

Chart review reveals that his left atrial size was markedly enlarged at

4.8 cm in the remote past. We will get an echocardiogram to be sure

there is no change. Clinically, there is not any heart failure. He

does not have any angina, and it is interesting that after this episode

when he felt his heart racing, he was able to walk extraordinary

distances. I doubt an ______ (6:31 knotted-hospital) myocardial

infarction.
