C5/C6 ANTERIOR CERVICAL DISCECTOMY AND FUSION - ILIAC CREST BONE GRAFT

AND ANTERIOR PLATE (BLACKSTONE)

British/Australian spelling used Discwise.

A horizontal skin crease incision was made on the right side of the

neck and dissection continued through the underlying *____ (s/l

perfusma profusma plathusma ) to expose the C5/C6 disc space. This

was identified using the image intensifier.

Hate to leave this one blank, it's the only blank in my first

discectomy. Anybody?

Could it be platysma? It is a platelike muscle that originates from the fascia

of the cervical region and inserts in the mandible and the skin around the

mouth. It is innervated by the cervical branch of the facial nerve, and acts to

wrinkle the skin of the neck and to deptrss the jaw. From Dorland's.

enough, to t.

His next op sounds like " exisions thoracics dechapsis " - makes me think of a

Thanks. I wonder if you are still working away. Anyway, I was happy with most

of the dictations I got this evening, but there are some mysteries I haven't

been able to solve. On a MAYO-KOONRAD TOTAL ELBOW REPLACEMENT (Koonrad? s/l

but can't find) he says An extra small 4-inch *-- s/l pumal component was

utilised. An extra small narrow stemmed ulnar component was utilised and a tip

was cut off to allow it to fit properly. The medulla of the humerus was

altrated with a Sulzerish plug. The s/ls are pumal, altrated and

Sulzerish/Sultzerisch.

s/l Excisions thoracics dechapsis - this is the operation, supposedly. I have

played with thoracix and abscess and all sorts. Can't get it. The text is no

help, just says

The posterior thoracic region was prepared, draped and infiltrated with local

anaesthetic. Methylene blue was infiltrated into this region to follow the

associated sinus. This was excised progressively after a vertical skin incision

through the previous incision.

The area was recurrently washed with Ringer's and hydrogen peroxide solutions.

Routine wound closure was performed after excising the skin edges using 3.0

Vicryl and 3.0 Prolene.

In an arthroscopy on shoulder, repair of subscapularis tear, he says what sounds

like

There was some chondral damage in the central posterior of the humeral head

which appeared more likely due to a chondral Hill-Sachs rather than arthritic

damage. There was also some moderate *___ chondral damage (s/l morta quake one

quandral damage) over the interior aspects of the glenoid. This was debrided.

Although there were good muscle fibres present there was very little tendon

attached to them. The best quality tissues which could be found were sutured to

the rotator in the *-- s/l fall or the rotator winnerfall-- and then to--- s/l

a frission or fissure--- groove in the lesser tuberosity with transosseous PDS

sutures. The repair was only moderately satisfactory because of the quality of

the tissues.

If it's obvious to anyone out there, I'd love to hear. These docs will be in

Pending overnight (way past my bedtime here in Oz)

Could you be hearing quadrangular cartilage here? If you have a Stedman's

Orthopedic book (or CD) you should be able to find the implants. I looked

in mine and found references to Mayo Clinic congruent elbow plate, etc.

Your " pumal " component could be humeral component, dechapsis could be

scapular, but I'm only making guesses here. Hope this helps a little to

point you in the right direction.

The closest thing I found was on this link that had information about a

type-3 " Kudo " total elbow prosthesis, and it states that it was used for

patients with rheumatoid arthritis. The Mayo part seemed to bring up the

Mayo Clinic. Here's the site I Googled in case it may help. http://www

About 10 hits down, it suggested a Coonrad-Mayo total elbow.

into Google again, one finds that there are a number of listings for

both a Coonrad-Mayo elbow and a Coonrad-Morrey elbow (which,

apparently, may have been modified by Mayo for their procedure; hard

Thanks again, Chuck. Another lesson in choosing one's words with care, in this

case for Google.

I am entirely confident of Coonrad. Thanks also Debi. I too was awash in Mayo

Clinic entries, having thought I was so clever in at least entering the Mayo

bit.

I'll go through my pending file again in the morning and enter that with pride

and gratitude. It will be so nice to send in my op reports with confidence.
