I would complain to the dr that gave you the referal that you do not like this dr (tell your dr. that he told you he wasn't listening to you) and that you refuse to see him again. I'm DEATHLY afraid he's trying to prove that I'm absolutely sane and then take away my lithium. He asked a couple leading questions about OCD, but I don't have it. There is a whole list of guidelines in documentation and you guys just have no idea how careful you have to be. First, he has to state exactly what the patient tells him. Is the statement he wrote false? If so, then yes, big problem. Maybe it would have been better if he had written patient stated " I started my child on vitamins and a week later he was talking " . Most often, parents won't say a direct statement like this, they'll elaborate, so you can see how hard that would be to put in direct quotes. He probably didn't tied the two statements together in the way that you're reading it. Remember, the doctor usually will transcribe his notes into a recorder directly from what notes he took in the office. Then a transcriptionist will type them out on paper. The whole idea of documentation, is to get the most accurate information into the smallest amount of words possible. I know that lay people often will read a chart and be totally offended to what looks to us as fair and accurate charting. An example: A miscarriage is often documented as a spontaneous abortion (legal term), a parent who would read that might flip out because of the word abortion. Next the doctor usually will chart his objective data. These are concrete things that he can smell,see, taste, and feel...concrete facts. A doctor may also put his interpretation of what he believes, or impression. This would give you the most information on what he is actually feeling or believing. They really need to make sure that the parents understand what is being reported and that it's accurate. Jacqui's evaluation looked a lot like what posted. And I understand that they are statements of fact. I also know that the transriptionist screwed up quite a few things. So, what is a parent to think? I'm not being defensive here or anything....LOL....but for $600, I want the report to project what really happened. The documentation must be factual, and can never be distorted to 'go easy' on whoever might be offended by reading it. Anyone who would have access to this chart would understand the rules of documenatation and therefore would know that when it says 'patient denied drug use', that it means just that. No medical professional would ever interpret that to mean " we believe the patient used drugs, but denies doing so " . If there was evidence or suspection of drug use, then that should be documented as well. Another example. Patient denies pain in affected area. That statement is factual. If I thought a patient was lying about this, I would document like this: " patient denies pain in the affected area, but grimaces when touched, says 'ow' and pushes away my hand when attempting treatment " Now that would indicate that the patient is acting stoic, or simply is not responding correctly to the question of pain. This situation very well could happen with an alzheimer's patient or someone who would be unable to verbalize correctly. My point is this. You do not want a medical professional to document anything other than this way. I can see why you might think he was trying to indicate something else there, but from how I read it, it was accurately documented. I'm kindof surprised that they would let you have your chart like that. Even my doctor won't let anyone just come in and read it, they must have a nurse there to help decipher it who would type more but can't cuz zach is screaming on my lap > Thanks .
