Maybe G5, P2, A3? Meaning she was pregnant 5 times total, 2 live births, 3

miscarriages/abortions.

Term help please

Under social history doctor states, " She is T5, T2, A3. "

I have never heard this before.

Yes it is G5, P2, A3. Is it unusual to have that under the social history?

Personally, if you're able to, I would put it under its own heading of something

like OB/GYN history. If you're not able to create your own headings, but you

can move things to the proper ones, you could consider putting it under the past

medical history.

I'll check some of his work samples and see what has been done in the past.

If I cannot find anything I'll have to leave it where it is.
