I did this on the advice from a social worker at the Patient Advocacy Foundation. You have to have a doctor supply you with a note, and whatever they require and a lot of other info, each person is different and requirements range, and some can be quick, some a long time, but if you go to www.socsec.gov then you should get all the info you need.
