We had a baby present in my Pedi Trauma ICU in need of a c collar. He wasn't really a neonate just really tiny. 0 collar didn' fit even with padding. Eventually our ortho folks fixed him up with a splint like bed. Out in the field how do we handle a neonate or premie in need of a collar? Inevitably, you'll have to use a SAM splint such as Lee suggested, or improvise a horse-collar arrangement using a towel rolled lengthwise. Unfortunatly, the amount of little babies we see in need of c spine managment is growning everyday. We initally did the rolled towels, that's how he was presented to us but the created a problem once we put him in the radiant warmer. Guest guest Posted January 5, 2009 Report Share Posted January 5, 2009 , Children's Medical Center Dallas Prehospital Committee has been looking closely at the use of " Evac Splints " to immobilize infants effectively. I have a picture of an infant brought in by DFR, the infant is so comfy, and immobilized he looks unreal. There are some teaching points if your medical director would like to approve the use of these for immobilization such as maintaining manual c spine until air is evacuated, and not impeding chest wall or airway. One of our physicians, is also working on research using this product for immobilization. The particular brand we have utilized is Hartwell medical. They also work very well for an infant that is maintained in a car seat to immobilize the head rather than a strip of tape, that seldom works.
