Guest guest Posted June 15, 2007 Report Share Posted June 15, 2007 I worked in L.A. and have been to this hospital several times. It is a pit. The place is grossly understaffed and very in adaquate. They shut their trauma center a couple of years back. As the County Health Department stated in 2004[ KING-DREW{ as it was known] IS A BLEEDING SORE ON THE HEART OF LOS ANGELES HEALTH CARE}There are more lawsuits pending against this facility, than any other hospital in all of California. This place needs to be bulldozed and all paitents in that area should be transported to St. Francis,which is a CHW facility that has level 2 trauma rating. How many agencies in our great state have any dispatch policies? Wes started this thread by throwing the " EMD " acronym into the Subject line...but this isn't about EMD...this is about policies that say WHEN and WHEN NOT an ambulance should be sent. How many agencies are dispatched by an agency they have no control over...say PD, FD, County SO, etc? If so, when do they decide to send an ambulance? How do you monitor that for compliance? Asking this because recently we have had some " anomalies " crop up with one of the 6 differing PSAP's that start the ambulance dispatch process. Once (about a month ago) an agency talked a caller out of needing an ambulance...fortunately this happened after first responders were enroute...so when they arrived on scene, we got a radio report...crew was a little surprised to be getting a report on a patient they had never seen....after pulling tapes, reviewing the call, the agency head saw no wrong doing on the dispatcher's part because " they were just following the wishes of the caller " ....not to mention the fact that the caller took over the call as the operator started the call...and the male caller stated that the female who had called for help was just over reacting to an argument they had had... Or maybe the caller who called after getting stung by bees wanting to know if she DROVE to the EMS Station could they look to see if the stinger was out and to make sure she was okay....Dispatcher advised that the EMS crew was out on another call so no one was at the station...but they should be back in about 10 to 15 minutes if she wanted to drive over then..... Our policy is ambulances go EVERYTIME, ALL THE TIME...but we are at the mercy of when our TIME starts...and I just bet this isn't a NE San area issue...I would suspect this is happening all over the state...nah...I'm probably just being paranoid... BTW, this leads to the next question...who can cancel or STOP the ambulance response...but I know nobody on here is canceling for the caller who called back 911 or the LEO on scene.... Dudley PS: Thanks Gene for stating the fact that EMS cannot commit an EMTALA violation...it doesn't apply to us (as long as we are not an arm of a hospital anyway)...but EMS agencies could surely be sued civilly for damages received by the hospital if the EMTALA violation occurred because we didn't communicate or work within the boundaries of the established " system " ... You find the patient in emergent distress and your protocol is to transport to the closest hospital (you are already there). You roll the patient into the trauma room and the patient is seen quicker because they are received by EMS. If your patient was found in emergent distress, then it worked out for the best and everyone goes home with a warm fuzzy feeling. In some areas I can see where transport to the nearest facility is required, but there are plenty of hospitals in LA county, and sometimes the closest one is not necessarily the appropriate one. This remains and sad situation, but the root is that many hospitals are abused by patients and the media. I agree, so lets make this a hypothetical case, lets say the facts as presented are exactly how they happened. What do you do then? You are called to the waiting room of an ER (a busy one) and find your patient as presented in the media. What now? The abuse of hospitals is without question, but that does not mean that anyone should die because of it. This is not a local problem, this is a nationwide issue affecting facilities, and ultimately patient care. As always, there is more to this story than the media is willing to find and disclose because it is not ratings worthy. In closing, I hope that people don't figure out that when they are not seen quick enough, they just abuse (call) EMS instead and call the media ..... Correct, there are many things left out, was she a frequent flyer? Maybe, but they get sick and die too. Food for thought, again, I am curious how this will all pan out. Guest guest Posted June 16, 2007 Report Share Posted June 16, 2007 I hope all EMS administrators and 911 administrators will pay attention to this problem and begin to formulate some reasonable and published policies on when, IF EVER, a call-taker can decline to send help when a caller asks for it. There is one huge gap, however, and that is where 911 is handled by either city cops or county sheriffs. Some do a great job, but some don't have the faintest clue about what they are doing. They use their civil immunity to hide behind when they make these errors. That is something that will require legislation to fix.
