Is that still the process? I looked at the DSHS web site and I don't see what I'd need anymore... Before I pick up the phone and call a field office, could someone point me in the right direction? This is the only application that is returned to the local office, or it was 2 yrs ago when I did mine. They use to have a course by course number that you could apply for, if you were only going to teach one class, not sure if that is still available or not.
