RE: High school students roster

Attention Coordinators and Instructors:

The form you should use to submit a roster of your high schools students

(who are not 18 and have completed your course) has been posted on the

DSHS EMS web site. It is listed under the EMS Education Materials as

Course Completion Roster for EMT and ECA. Please use this form when

submitting a list your High School Students who have not reached 18

years of age as verification of their course completion.
