Division Chief of Training Hyde was ultimately responsible for the alleged safety lapses that led to the death of Racheal , 29, Dixon said at a 12:50 p.m. press conference, speaking only hours before 's scheduled burial in Denver, Colo. Lt. ph Crest, the incident commander on the day of the exercise, and Lt. Larry Broyles, the lead instructor that day, were suspended without pay for 60 days. collapsed Feb. 9 during an exercise involving a city building deliberately set on fire. She died at a local hospital of thermal injuries and asphyxia, the medical examiner's office said. Two other firefighters were injured when they went to 's aid. She was one of 24 recruits taking part in the exercise and was on the third floor with two other trainees responsible for punching holes to ventilate the building. Sources told WBAL TV 11 News that several fires had been set within the building, even though safety regulations call for only one fire to be set. Fire Chief J. Goodwin Jr. said the preliminary report on the exercise " is really, really not favorable for what happened that day. " Hyde didn't follow procedures, Goodwin said. The abandoned West Baltimore home was approved for a fire-free training session where firefighters perform tactics such as using a pike pole, Goodwin said, but " it was never chosen for a live burn. " Dixon said reading of the lapses in the preliminary report left her " angry and shocked. " The report will be made public Friday, she said. She's also asked an independent panel to review the incident and make recommendations by March 30. The city's training academy was reaccredited by the state the week before 's death. " Why this went bad, we'll get to the bottom of, " Goodwin said. He said the exercise may have been " a little more than a rookie should encounter.
