cheat sheet question

Does anyone have a source for the little " cheat sheet " laminated cards that some

of us wear behind our ID cards?? In particular, I'd love to find ones with GCS

scores and the 12-lead/STEMI correlations (i.e. location of the infarct in

relation to the 12-lead findings).
