If you research Boston and find good >things then systems the seize of Boston need to emulate that and see if it fits >but?it's not likely gonna fit in Flynn, TX etc. All true..however, there can be a baseline (base not ceiling) standard for EMS. True and agreed (great minds again?) but I think that is a separate issue from the problem of " this this study said XYZ about X and therefore we all need to do XYZ about this now " . I have absolutely no problem with say that An EMT-B does this as a base or an Ambulance is defined as this or that in terms of basic equipment and the like. Iowa has begun that process and will be piloting the project next year. A task force was formed a year ago with the goal of developing EMS system standards. We began by attempting to wipe out the thoughts of " this is how WE do it now and we are awesome " . Not to mention the ever present " my system works well, why fix what isn't broken? " Every system has those that think they are the be all end all and at times that last line has a bit of merit but not over the long haul, systems must evolve. We set out with a clean slate... " IF you had to build a system from scratch, where there is nothing, what should it look like? What should every single person who drives through, lives or works in Iowa be able to expect from an EMS response, regardless of where in the state the event takes place? " Interesting premise. There are places in Texas that today likely have no EMS System nor really a need for one since there are more cattle then humans in the area but Texas is a HUGE state and we have areas of HUGE growth and therefore what is barren plains today might be suburbia in 10 years so this has merit here as well. There can be a baseline standard for EMS..it isn't one size fits all, but every single person who calls EMS should be able to have a reasonable expectation of the basic care they receive. Ah but if one a person chooses to live 300 miles from a phone can one get phone service? It's a matter of balance. My bosses own a Ranch in Collingsworth county, great place to go and chill but since a phone line would cost 5 grand plus they opt not to have one, 17 miles to even get a shot at cell service. i go there I know that and I accept that if I have my MI while alone there I will likely die there. Unfortunately, too much knee jerk reaction takes place to research, which as stated, does usually have some bias or slant of the researcher. Until research is no longer conducted by humans, that is simply a fact that has to be considered. We could let Firefighters do EMS research (I can say that as I is one). Mongo lives. Louis N.
