Have your PCP send it in again or ask for all your paperwork and hand deliver it yourself. I spent 4 entire days making phone calls tracking down the exact kaiser fax my PCP was supposed to send it to, and finally calling patients assistance. Patient's assistance really helped..my paperwork was " found " that week...plus they had the second set sent by my PCP. I was so angry that SSF Marina..transferred me to Richmond and I had my surgery 3 months later. It takes persistence at this point. Call them everyday, several times if you have to.
