Guest guest
I am not sure I posted this correctly. let me try Again. I was wondering if anyone where I might find samples of the various types of letters Dr. R. is looking for. i found the appeal letter, but was wondering if any Psych. Support, Family Support, PCP Support, Patient Letters have ever been posted anywhere. I could use some guidance. Thanks, Dave
Guest guest
Dave, I don't think there is any samples of letter that you are looking for. They support letter need to come from you family and friends hearts. The psych letter you are looking for would only come from the Dr. that you have seen. You PCP should know how to write a support letter of what he/she is referring you for. The contact letters that you are talking about are from 10 people that have had this procedure done. It was really hard for me to contact people, I am a shy person, but I knew I had to do this. I called people, and some wrote to me. I wrote down some questions that I thought I needed to be answered and asked each one of them. In about a two page letter I documented what they had said to me. I know this is not a lot of information, and I wanted the help also, but someone told me you have to do this yourself. You have to understand what is going on. So, I did it. If you would like for me to be one of your contacts let me know. MGB 2-9-
Guest guest
Thank you and you were a great deal of help. I am overly cautious at times and I get to wondering if I have fully covered the bases properly. I suppose every PCP, Psych, and family member will write something different and worrying about it will not help. Than you for your thoughts on a contact letter. I did not know how to document my contacts and was simply going to write down their names and the mode of contact (phone, email, snail mail etc.). I never even thought of writing a letter separate from my Patient Letter. Thanks again for your help. Every bit of input helps. Caldwell Technology and Media Services Coordinator/ S.P.A.S.
