Many of us use or have used compounding pharmacies to avoid various excipients in our meds. I am visiting a relative in Florida. A very sad event just happened here when 21 polo ponies were given injections of a vitamin preparation prior to a competition. All 21 of these very valuable horses fell ill suddenly and collapsed at the preliminaries.....all 21 died. Reports stated that each pony was worth over $100,000. The compounding pharmacy was known in the area and certainly the owner of these horses was careful with his valuable investment. It is reported to be a mistake at the pharmacy, pure and simple. Selenium was added at 5 mg. rather than .5 and the error was fatal. This was determined when the state lab did testing on the horses. Everyone involved is distraught as no one considered something like this could happen. I don't know what controls are in place when drugs are compounded rather than produced in bulk as each Rx would e made up individually. Human error is always possible.
