Got the names and telephone numbers (FAX and e-mail addresses, also) in both the employers benefits office and the BX-BS office. I checked in daily with everyone; just said " hi! " and chatted a bit. Nothing heavy. Sent thank you cards after approval. I've been sending them cards and e-mails detailing my progress since the surgery six weeks ago...and thanking them again for their assistance. Guest guest Posted February 3, 2000 Report Share Posted February 3, 2000 , I have BCBS PPO Plan and what they had to have was medical records for 5 years showing a history of obesity and that you had tried to lose weight under medical supervision. I had taken Redux in 1996 and had gone to Dynamatrix which took blood tests and one examination by a doctor. That did the trick. Of course the most important thing is if it is covered in your insurance benefits. Mine read this way: Exculsions and Limitations For any treatment or regimen, medical or surgical, for the purpose of reducing or controlling the weight of a member, including treatment of obesity, except for surgical treatment of morbid obesity; Once by my medical records it was deemed a medical necessity, then the last part of the above clause got the approval. After my surgery though, Durham Regional billed them for the wrong diagnosis and they denied payment. What a scare! After it was re-submitted with the correct diagnosis for the MOB it paid with no problems.
