I just ordered one online at _www.AmericanMedical-ID.com_

I paid $38 (for stainless steel bracelet.) I paid an extra $7 to have it

done next day rather than 2 to 3 weeks, and $6 for shipping. No cost to engrave.

I had the front and back engraved because there was so much to put on it!

On the front I had:

Dana Lynn Colan

In emergency call

KSSF (650)742-2000

Ask for Bariatric

Surgeon on call

On the back it will say:

Roux-En-Y gastric bypass

No NG tube,use endoscope

No NSAIDS.No Aspirin

No Aspirin derivatives

One thing I thought of is that I put my blood type on the front of the dog

tag with my name, all the other info was able to fit on the back........Just

a thought for anybody else getting one.
