I'm not sure if I'm jumping in on the middle of a conversation, but....when I

got my manual in the mail (two copies actually), it had two pages of contacts

stuck inside the back cover. Is this what you're looking for? The contact

list has name, city/state, phone and email addy as well as date of surgery.

I think that's probably it, . I downloaded the manual and the list isn't

on there.
