Hi,

If you are a patient of Milhorat's and have not filled out a form to request

your records to be copied from last location to his present, you may want to

do that as soon as possible. Call his office for the form.

It takes a couple of weeks for this transfer and if you get sick quickly

(like we do seem to do) it is hard to wait a few weeks for the records.
