First sonogram (gyne)

11.05cmx6.84cmx6.97cm (lxhxw) endo thickness 1.44cm

Observations read: uterus is enlarged; heteroggenous

masses and hypoechoic masses. The endometrium was

difficult to see due to mass. Left ovary contais

Follicles. Right ovarylarger than left and contains

Follicles.

Comments: TA and TV exam. Uterus contains heteogenous

mass mid body 4x4x4cm possibly consistent with fibroid

or adenomyosis. Other hypoechoic masses; RT body

3.3x3.3cm, Fundus 12x9.7mm probably fibroids.

(note the diagnosis of adenomyosis here...I was only

told about this possibility seven weeks ago)

A) Endocervical Tissue:Benign reactive endocervical

and gladulzr changes associated with acute and chronic

inflammation.

Endometrial tissue probable fibroid biopsy: Benign

inactive lower uterine segment fragments associated

with portions of smooth muscle. sections demonstrate

benign superficial lower uterine segment tissue

associated with bands of benign smooth muscles fibers

suggestive of a leiomyoma. (I have a picture of this)

C) Endometrial tissue: Benign, showing inactive

polypoid endocervical/lower uterine segment

fragments...suggests a polyp (I have a picture of

it...it is a polyp)

Uterus is anteverted. It is markedly enlarged

measuring at least 13.7x8.7x8.9cm. The uterus

demonstrates diffused inhomogeneous acoustical

architecture constistent with fibroids. (Doc was

interested in blocking arteries...so they talk about

blood flow but could not see well)Right ovary measures

6.3x3.5 cm. Left ovary cannot be demonstrated

(great now we lost the ovary in addition to the test

results)

Ultrasound post myo: anteverted uterus measures

12x8x8cm. heterogenous area of altered echogenicity is

noted within the fundus of the uterus 5.7x3.9cm. A

similar slightly larger fibroid was noted in the

uterus on the previous study. Right ovary

4.2x2.6x4.2cm and contains multiple physiologic cysts.

Left ovary measures 3.8x2x1.3 cm and also contains

physiologic cysts. Minimal fluid is noted within the

cul-de-sac.

Impressions:enlarged uterus...the fibroid displaces

the endometrium posteriorly. The endometrium is at

the limits of normal thickness. Tehn talks about the

cysts in the ovaries again.

5/27/ 03 taken off the Megace to wait for Lupron

5/29/03 later bleeding spots

5/31/03 hemorraging...5 sets of clothes changes

6/1/03 continuing still waiting for the Lupron

6/6/03 finger stick hemo 10.1 at general practitioners

office

6/11/03 Lupron 7.5 mg and back on Megace

6/13/03 finger stick hemo 9.7 at general practitioners
