The Florida Center for Inclusive Communities is conducting a survey to

determine the needs and issues confronting Floridians with Developmental

Disabilities in the areas of education, community living, employment, and

health.

This survey will help us understand your issues and concerns and help us

plan the activities of the Center. This survey is very important to us.

Your opinions are important to us. The survey will only take 5-10 minutes

The Florida Center for Inclusive Communities (FCIC) at the University of

South Florida was established in October 2005 through a University Centers

for Excellence in Developmental Disabilities Education, Research, and

Service grant award from the Administration on Developmental Disabilities.

The FCIC’s mission is to provide training, technical assistance, research,

systems change, and dissemination activities focused on supporting

individuals with developmental disabilities to:

- Make informed choices and decisions about their lives.

- Receive support and services that are based on individual goals and

outcomes.

- Achieve full inclusion and participate in society, in an individualized

manner, consistent with the unique strengths, resources, priorities,

concerns, abilities, and capabilities of each individual
