During the discussion and plan, the doctor says the following: Portal hypertension, secondary to neoplastic liver infiltration. He is on Lasix plus Aldactone. We will monitor his mental status and give lactulose as needed. Mental status doesn't seem right here but that's what it sounds like he is saying. He is an ESL, but a very careful dictator.
