Medicaid question

Hello,

If a child is covered by FL medicaid that she/he is eligible by being on the

waiver, and they move out of the state, what happens? Is she still covered for

Medicaid services? Does she need to go to the Medicaid office in her new state

and fill out forms? Is the waiver unique to Florida?
