The next step as his liver failure worsens is for all those symptoms and signs to get worse, unfortunately. The ascites will get worse. The bleeding from his gums and any sores will get worse. The varices will get worse. The swelling of his feet and ankles will get worse. About the only good thing will be that his MELD goes up. A low animal protein diet might be of some help reducing the risk of Hepatic Encephalopathy (confusion and coma related to worsening hepatic failure). I'm glad 's being evaluated for tx as it's obvious he's had PSC for quite a while. Best of luck.
