I was facing many of the fears and uncertainties that you are, particularly with my first PG, where all I could get out of my doctor were a bunch of " probabilities " but no clear answers. Being the proactive sort, I actively lobbied for a cerclage and was told that unless it's indicated (due to past losses, cervical shortening or some other factors in a woman's history), then the risks outweigh the benefits and my OB would NOT go there. I did not actually have a cerclage in any of my pregnancies, so I don't have first-hand experience but in my extensive research, I've read of having it placed anywhere from 12-14 weeks, if it's " preventative " due to history. I've also read that cerclages tend to be more effective if they are placed at the onset of changes or problems, rather than later, when they are more of an emergency intervention. There are several women on this list who did have cerclages, or are currently PG with one in place and it seems that they've served our members well, I believe most went to full-term before having the stitch removed. I know the thought of bedrest etc. seems daunting, but try to think of it as your doctor being conservative and extra-cautious - I know that with MA's, including UD, this is a better (and safer) approach than the typical " let's wait and see " that many clueless OB's practice, sometimes to the detriment of high-risk patients. When I went for anatomy, they found that my cervix was open and the remaining was 1.2cm. However the high risk doc were watching it before and it was 3.6cm at 14 weeks. SO I had the stitch and modified bed rest (only bathroom and shower). At 19w the length was 1cm. Before I go on, let me tell you that found out that I have a septate uterus and they did the procedure last week. I just want to mention that I had this problem as well so you know why I did not a great result from the cerclage. Everything was fine but at 20w I had some bleeding and got hospitalized and unfotunately lost the baby at 21w. So, if I were you, even though you are not prescribed bed rest yet, I would take it easy and maybe try to remove pressure from the uterus as much as I could. There are many people with preventive cerclage, which experiencd a very successful pregnancy and agian mine was the shape of uterus as well as incompetent cervix so be POSITIVE and enjoy the sex of baby, finding name and etc. Wish you and the baby the best Zohreh, 2 losses - SU(resected)/IC --- In MullerianAnomalies , " Hobbs " We went for the fetal anatomy today at 18.5 weeks. Everything looked good and it is a boy. So we're sitting there waiting on the now routine every 2-wk transvag us for the cervical length talking about names, nursery changes, etc. Then when we get the tvus, the cervical length is 25mm now (down from 30mm (we think it was 28mm at the last check)). This is a test that helps to predict if preterm labor is likely to occur within the next two weeks. The test is similar to a pap just without the lubrication. A negative result is a very accurate prediction that labor is not likely to occur while a positive result is less accurate. Let me know if you go back to work. If not, we can be bed rest buddies! I am currently 36+ weeks pregnant. I have UU and IC. My cerclage was placed at 17 weeks. My cervix measured great until 15 1/2 weeks...at that check it had gone from 3.9 to 3.2 CMS. The key for my Dr. was, not the length, but the amount it had shortened in just 2 weeks. I hope this helps.. By the way, I have a modified Shirodkar cerclage....which is higher up on the cervix. Once it was placed, I spent 3 weeks on bedrest. After that, I've been on a no long distance walking, no sex, no heavy lifting etc....routine....and have been just fine! Good luck to you both!
