Dear, Volunteer EMS Departments Wind Foundation has will be selecting an EMS department to be a candidate for our services. In order to see if your department qualifies we will need to get some information. Please answer the following questions and mail them back to me. Or, email them back to me at.: windfoundation@... Please send the following if you have it. 501 © (3) By-laws board of Directors Who is your governing body? Were do you get your budget, state, county local water tax? If you receive funds from an outside source, would this reduce your current budget? And would it affect the following year's budget? What size is the district in which you service and how many calls in a month and total for the year 2004? IF you had three areas that you could get help with, please list them. In your budget is there and line item for vehicle replacements if so how much and how many years before you get a new unit? In your budget do you have a building plan? If so how much, and what year will you be able to start to build? If no building plans, please submit why there is not a plan. How many paid staff do you have? How many Volunteers do you have? What is your yearly budget? We need an official person to sign all paper work name of this person phone, cell, email fax number and an address.
