Dear Rhonda and all,

IT is great that more or you are putting together your own MR. (medical

record). We spent time talking about this at the symposium. When it comes to

records re: x-rays, films, CT scans, MRI, ERCP, etc., it is important to have

the radiologist's official report.

I suggest making a cover summary page listing:

1. test

2. date ordered

3. purpose

4. place exam done

5. address and phone where exam done

6. name of ordering physician

7. contact information for physician

8. results of test

A sample of a complete medical record that every patient should keep for

themselves will soon be available on the home site.

Briefly, regarding medications, the PAI consulting physician stated that the

list of current as well as recently discharged medications should include

everything found on the label:

1. Date ordered

2. name

3. strength

4. amount

5. frequency

6. route

7. duration (if applicable)

8. purpose

And ordering physician's information, i.e.;

9. name

10. title

11.office number.
