For me, the decline from around 40% to tx took almost 10 years. While your grandson's mileage may vary, with an agressive treatment he could hold above tx level for some time. My neph and I did not even bring up the " t " word until I was under 25%, and most insurance plans do not think it is medically necessary (aka " a covered event " ) until much lower than 37%.
