I don't have any reference to give you. I've just always seen the nick-name

printed like a middle name, so I would put:

Siegler, J.J. " Kip "

Hi all,

I have a patient name (this is on a non-graded assignment and is a

printed publication so confidentiality doesn't apply here) that

contains a nick-name like this: J.J. " Kip " Siegler.

Does anybody know where the correct way to do this on a chart with

last name first might be found in the BOS? I tried names and titles,

neither of which gave a clue.

Any help would be MOST appreciated!
