Guest guest Posted July 23, 2004 Report Share Posted July 23, 2004 This is one of those marginal type of things, that is, do we take an ACE inhibitor even though we don't have high blood pressure and the proteinuria is mild? There is no good answer, because it ranges from the don't take a medication unless it's really necessary camp, all the way over to the prescribe an ACE inhibitor and some fish oil in case it might help camp. It's getting to be more and more common to prescribe an ACE inhibitor fairly early, even in the absence of high blood pressure. Will it prevent progression of the disease? I don't know. I think it's fair to say that recent evidence suggests an ACE inhibitor can be helpful if there is proteinuria, even though the person may not actually have high blood pressure. On the other hand, rarely, these drugs can increase the level of kidney failure, so I think it's important to monitor this closely when prescribing it. Sometimes, they can also cause anemia, to some extent, or make it worse. Like any drug, your doctor has to weight the possible benefits against the possible risks, and monitor appropriately. I have never had high blood pressure, in fact it has always been fairly low - now it's normal 110/70 - yet I've been on accupril since I was diagnosed October 2003.
