I believe some require a Medical Durable Power of Attorney. Whether it needs to be notarized varies from state to state. I have a document called The Five Wishes, and I believe its a great document which allows you to spell out more than just what circumstances you want treatment stopped or when to be declared a DNR. It's only accepted in 40 states though. In PA you need 2 witnesses not related to you nor your proxy/advocate to sign it. You can find out more info at: _http://www.agingwithdignity.org/5wishes.html_ (http://www.agingwithdignity.org/5wishes.html) I believe an individual copy costs $5, but I was able to get a free copy from my pulmonary social worker at CHOP. Whether you decide to use the Five Wishes or write an advanced directive, it is still very important to talk with family members about your wishes and decisions. Even with these documents in place, if family members are conflicted about what you may need or not need, hospital personnel may still do everything to prolong your life, because of threat of a lawsuit by an unhappy family member. Advanced directives or not it is very difficult for family members to make these hard decisions. had named me his proxy, which didn't go over well with his mom at first, and I was fully aware of his wishes. We had discussed it a lot, but it was still difficult for me b/c there can be gray areas. I can't imagine how I'd feel if I was totally clueless of his wishes. One thing I do highly advise is have more than 1 copy out there of the document. I wanted a copy of 's but his mom had gotten a hold of the only copy, and she wasn't very corporative. She never got around to getting me the copy. Unfortunately she also wouldn't allow even his nursing agency to have a copy, she believed it was private info (do not ask me why) and all they needed to know was he was a full code. So of course once we needed it, she couldn't find it. I was lucky CHOP knew we were engaged and honored his wishes to have me make the decisions, but his mom also did not fight me. They do not provide very specific information (she " wanted to spare the details " ) and brushed off my expression of 's wishes. I asked if was able to use the laptop that was provided to her and the nurse replied " we are able to understand her needs, " leaving me to believe that they are still not communicating with beyond her requests to reposition. The nurse did report that 's vent O2 level is down to 40%. They attempted to extubate her today but " she didn't do well on the wean test. " She would not describe what this wean test involved and became very defensive when I asked if they were familiar with Dr. Bach's protocol or had communications with him specific to - stating that medical care decisions are left up to her parents and I can discuss those specifics with them.
