To EMS Providers that participated in state initiated missions during

hurricanes Katrina and Rita,

Several weeks ago, the department sent out information concerning

reimbursement issues related to responses to hurricanes Katrina and Rita. In

those letters, two deadlines were mentioned.

The first deadline was for the Applicant/Provider Agreement, which was due

by January 6, 2006. That deadline has passed, but participating firms are

still encouraged to submit that signed form as soon as possible. Please

understand that the forms that were received in a timely manner will be

processed in a higher priority.

The completed reimbursement packets were to be submitted by the second

deadline, which was today (January 20, 2006). We are aware that many of you

that met the first deadline for submission of the Applicant/Provider

Agreement have not yet been contacted by your Public Assistance Coordinator

(PAC). Department officials will be attending meetings on Monday to attempt

to expedite this process. Please expect additional information from the

department on this issue early next week.

IMPORTANT

Your January 20th deadline has been extended to allow time for the PAC to

contact you and provide assistance with your reimbursement packets. We

apologize for the inconvenience for this delay even though it has been

beyond our control. Please do not submit any additional completed

reimbursement packets to the department until you have received additional

instructions from the department.

Once again, the January 20th deadline has been extended pending further

instructions from the department. Additional information will be sent out on

the e-lists early next week.
