Jane,

I just pulled a number out of the air for an " average " charge. Since we have

agencies still billing one of 4 different methods, I didn't want to confuse it

with any more details than necessary.

I think I replied later, our average, fully-loaded call is approximately $850.

That includes base rate, mileage, and supplies.

It has been said over and over again..... As long as EMT's and Paramedics

are willing to work for low wages, and or volunteer their services they will not

ever receive the money they are so long over due.

Think about it. I refuse a 12 an hour job. Rookie Paramedic comes over and

takes it. Therefore owner did not have to raise his pay rate.

That is an excellent point, and one that seems to be constantly

overlooked. I once met a guy with the State Fire Marshal's office. I

told him I thought that must be a pretty cool job. He said, " Not

really. The cops don't respect you because they think you're just a

fireman. And the firemen resent you as just a cop. " That is exactly

what is happening to EMS.

Let's face it, the only reason that EMS ended up as a part of the fire

service to begin with is because bean counters thought that

firefighters weren't doing enough to earn their money. " Hey, those

guys are sitting on their asses anyhow. Let's make them run the

ambulances! " Consequently, EMS is forever tagged as a sideshow, not

quite as worthy as the main event.

Once you take away all the silly patches, badges, pins and epaulets,

what makes EMS a part of the public safety sector? No matter how long

I ponder this question, it still comes down to one thing: lights and

sirens.

It's time to cut the umbilicus and grow.

Your comments bring up a great point. IF you have not read the EMS agenda for

the future...go to http://www.nhtsa.dot.gov/people/injury/ems/agenda/emsman.html

and read IT.

This scope of practice is being built for an EMS system that does not currently

exist. You have to read the Agenda to fully grasp this...but one of the reasons

we cannot fathom the need for or how this Advanced Practice Paramedic thing will

work is because we are trying to put it into our current EMS model.

The Agenda lays out an entirely new EMS system where transporting patients to

the hospital is only ONE of a NUMBER of things that EMS does including primary

care, public health activities, referral work, etc....transport of patients

becomes a much less piece of the picture where now it is all we see.

Once you see this it becomes a little clearer as to why the Advanced Practice

thing is the way it is...but again...no one is working on the EMS Agenda for

Getting Reimbursed in the Future....
